With the Mobile App, you can check the history of your driving record in the last 2 years for free.

Despite that, a certified and uncertified MVR report (3-year and 7-year history) is also available to you. Payment options that all individuals can use are Visa, Mastercard, American Express, and Discover credit cards.

You can find the app on both, iOS and Android devices. If you are an iOS user, you can find the app in the App Store, On the other hand, Android users can find it in Google Play.

On the Internet using Online Services

After creating an Online Account, people can request a 3-year or 7-year uncertified or certified MVR record. Payment options for the Internet method are valid Visa, Mastercard, American Express, and Discover credit cards. You can also pay by mail. A payment coupon will be available for print at the end of your transaction. Check, cashier check, or money order are accepted, as well.

In-Person at any DDS Customer Service Center

The person can request either a 3-year, 7-year, or Lifetime MVR report. Complete the necessary Motor Vehicle Request Form (Form DDS-18). The form must be original, so the faxes and copiers are not acceptable. If you decide on this method, there are a few things you should know

• The personal data you need to fulfill are the full name, date of birth, and Georgia driver’s license number of the licensee.
• Remember to bring valid identification.
• For requesting an MVR for another person, you must have that person’s signature.
• Payment methods that are available to you are Cash and Credit/Debit Cards including Visa, MasterCard, Discover, and American Express. Note that checks, money orders, or cashier checks will not be accepted.

By Mail

For requesting a 3-year, 7-year, or Lifetime MVR certified copy of the Driving History (MVR), you have 2 options. You can submit a letter or complete Motor Vehicle Request Form (Form DDS-18). The request should include the following:

• Your full name as it appears on your driver’s license,
• driver’s license number,
• date of birth,
• complete mailing address,
• and self-addressed, stamped envelope.

Mail it to the Georgia Department of Driver Services. MVR Request, Post Office Box 80447

Conyers, Georgia 30013. In case you are requesting an MVR for somebody else, do not forget to get that driver’s signature.

Notations

Non-Certified Driving History Report (MVR)

The uncertified driving history report is only available for viewing. That means you can not print or download this document. After you request the non-certified MVR, it will be available for 30 days.

Certified Driving History Report (MVR)

The certified driving history is intended for employment purposes, insurance agencies, and others that request a version stamped and certified by DDS. You can not view the document online, but it can be printed, certified, and mailed to your address. The third person can get this report if it is on behalf of the customer.

Fees

There are three types of fees you will have to pay to complete the process

• For 3-year Driver History Report the fee is $6.00
• For 7-year Driver History Report the fee is $8.00
• For Lifetime Driver History Report the fee is $8.00
